[SBC] Clarendon Hill Bicycle Accident

Has anyone heard of the bicycle accident with a truck on Clarendon
Hill in December? I can't find anything in either the S'ville News or
Journal. I found the following blogs:

The blogs have sparse follow up info. I would be interested in the
outcome, but also why it happened to see if there is anything the
Bicycle Committee could do to improve the situation, or atleast raise
awareness of a specific problem.

To that end, Steve, does the city have an bicycle accident database
that we can access or get a copy? I think knowing where past bicycle
accidents have been and what caused them would be helpful. I know some
of this information (i.e. location) is available in the Safe-Starts
report (or Safe-Streets, I forget the title), but I'm more interested
in a full accident report that we may be able to determine why the
accident happened and try to address the cause.
