[*BCM*] Anyone Have Experience With The Comm. Of Mass Registry of Motor Vehicles "Complaint of Improper Operation" Form

Has anyone used this form and followed up on it.  I had an ugly experience
(in the car) yesterday and I want to see if I can get some degree of
satisfaction.  I'm told this applies for bike-to-car experiences, too.  If I
used this when I rode, I'd be filling at least one out each hour I was on
the road.
